Cover Page
On the Cover Page, you report general information such as your name, email address, position/title, grade, etc. This is to identify to the reviewer who you are and, to some extent, what you do. You'll want to read the instructions for Parts I through IV before you start filling out the form.
You'll notice that the cover page includes a list of questions that ask whether you have any reportable assets, income, liabilities, outside positions or agreement or arrangements. If you're not a new filer or a "special government employee" (appointed to temporary duties not to exceed 130 days in a 365 day period), you'll also need to answer whether you, your spouse or dependent children received any reportable gifts or travel reimbursements.
If you answer "yes" to any of the questions , you'll need to complete the rest of the form.
If you answer "no" to all of the questions, all you need to do is sign and date the form and submit it to your ethics office.
OGE structured the form in this way so that if you don't have anything to report, your ethics office will only need to store the cover page of your report.
