IMPORTANT NOTE - PLEASE READ BEFORE ACCESSING OR DOWNLOADING FINANCIAL DISCLOSURE REPORT FORMS

The financial disclosure report forms, both the OGE Form 450 and the SF 278, are provided below in various versions that are suited to individuals with different reporting requirements. Please read the instructions fully before choosing the appropriate form for your own needs.

 

FOR USE STARTING JANUARY 1, 2007

Confidential Financial Disclosure Report (OGE Form 450)

IMPORTANT:  Please do not send your completed OGE Form 450 to OGE.  Your agency, not OGE, is responsible for reviewing and maintaining the forms for its employees.  Contact your agency ethics official to obtain the correct address for mailing your form. 

Provided below are two enhanced versions of our electronically fillable OGE Form 450 PDF form. One version meets the accessibility requirements of Section 508 of the Rehabilitation Act Amendments of 1998 (Section 508). The second version is a non-accessible version of the electronically fillable OGE Form 450 PDF form. The programming for both versions of these electronically fillable forms has been revised to allow users to insert more information in various data fields, generate automatically unlimited additional pages for Part I, Parts II and III, and Parts IV and V (only available in the non-accessible version), and save and reuse a completed form without having to purchase Adobe Acrobat software. Through our purchase of special licensing and software from Adobe, users can now save a completed form and generate unlimited additional pages by using the latest version of the Adobe Reader, Version 7.0, which is available for free from the Adobe website ( www.adobe.com ).

OGE Form 450 – Electronically Fillable Non-accessible PDF Version of OGE Form 450 (2007)

IMPORTANT:
Although this version of the form is capable of accepting digital signatures, OGE has not authorized any agency to accept forms with digital signatures except those participating in an OGE-approved pilot program. For the 2007 filing season, each filer must place an actual signature on the form and submit a paper copy of the form. In the future we hope to authorize the use of digital signatures and for that reason designed the form for this future use.

Filers will notice that the form has been substantially revised. It is critical that you read through the entire form, gather your financial records that contain the information requested by the form, and then fill out the form.

This revised PDF version of the form has been programmed by OGE to be filled in electronically through the use of the Adobe Reader Version 7.0 and then printed. Users may now use either the free Adobe Reader Version 7.0 software or Adobe Acrobat software in order to electronically save completed PDF versions of the OGE Form 450 or to generate additional pages.

Public Financial Disclosure Report (SF 278)

Provided below are two enhanced versions of our electronically fillable SF-278 PDF form. One version meets the accessibility requirements of Section 508 of the Rehabilitation Act Amendments of 1998 (Section 508). This version was programmed in a manner that makes it accessible to those persons with vision impairments. The second version is a non-accessible version of the electronically fillable SF-278 PDF form that does not use the current screen reader technology. The programming for both versions of these electronically fillable forms has been revised to allow users to insert more information in various data fields, generate automatically unlimited additional schedules (only available in the nonaccessible version), and save and reuse a completed form without having to purchase Adobe Acrobat software. Through our purchase of special licensing and software from Adobe, users can now save a completed form and generate unlimited additional schedules by using the latest version of the Adobe Reader, Version 6, which is available for free from the Adobe website (www.adobe.com).

Important Note: It could take approximately 20 minutes to download the nonaccessible version of our electronically fillable SF-278 PDF form if the user does not have a high-speed Internet connection. Because this may be impractical for some users, we are providing a link on our website to a Department of Defense website that has an electronically fillable Microsoft Excel software version of the SF-278. This Excel version will take users less time to download but will require users to have Microsoft Excel on their computers to complete and save the form. It will not, however, have all the features of our enhanced nonaccessible version.

As other electronically fillable versions of the SF-278 form become known to us and are posted on the Internet, we will provide links on our website to agency websites that have the form. Users may then choose the electronic version that they find most suitable to them.

Notes:

  • Printed copies are currently available through the General Services Administration Customer Supply Centers.
  • The InternetForms Viewer is no longer available.
  • Instrs., Privacy Act Statement. There are now three additional routine uses as well as those listed on the SF 278 form (see DAEOgram DO-03-015 (PDF - HTML - TXT) of August 25, 2003 (p.2) for more details on the new uses.
  • Sch. B, Part II. The reporting threshold for gifts and travel reimbursements received from one source is now further increased to more than $305, with a $122 or less de minimis aggregation exception (see OGE DAEOgram DO-05-007 (PDF - HTML - TXT) of March 17, 2005 for more details on this change.

Robert Cusick, Director of the Office Government Ethics (OGE), opened the 15th National Government Ethics Conference by discussing OGE’s Leadership Initiative.  Mr. Cusick's remarks focused on OGE's commitment to raising the level of discourse between the leadership of executive branch agencies and the ethics community.

Robert Cusick gave the keynote address to open the Defense Industry Initiative's Best Practices Forum for 2007 in Washington, DC June 14 and 15.  The Forum attracted approximately 250 participants representing numerous defense industry contractors.  Mr. Cusick's remarks focused on the issues resulting from the increasingly blended government employee/contractor employee workforce.

Ethical Implications of Emergency Response Conference Report (June 2006)  (PDF)

This report is the result of a 6 month effort of a working group comprised of staff from the Office of Government Ethics and numerous ethics officials. The working group was initiated in response to hurricanes Katrina and Rita and ethics officials request that the community come together to develop common solutions to shared problems and experiences. The effort culminated in a conference that was held March 21 and 22, 2006. In addition to the conference report, many speakers prepared Powerpoint presentations which are listed on the report.
