Potts Director SUBJECT: Annual Agency Ethics Report Please complete and return the attached questionnaire to the Office of Government Ethics (OGE) by February 1, 1994. Your response to this questionnaire will constitute your annual report for 1993. Section 402(e)(1) of the Ethics in Government Act of 1978, as amended, requires that each executive agency submit to OGE an annual report containing information about the agency's ethics program. As you will note, the content of the 1993 questionnaire is similar to the 1992 questionnaire. The changes specified in the October 15, 1992 memorandum, that transmitted the 1992 questionnaire, have been made. Moreover, we have included questions in the EDUCATION AND TRAINING, PUBLIC FINANCIAL DISCLOSURE, CONFIDENTIAL FINANCIAL DISCLOSURE, and SPECIAL GOVERNMENT EMPLOYEES sections that concern reporting the reason(s) why: (1) the number of employees who were required to attend ethics training was not the same as the number of employees who actually attended ethics training, and (2) the number of financial disclosure reports (i.e., public and confidential) that were required to have been filed was not the same as the number of reports actually filed. Please note that your submission must include a distribution of confidential filers by grade and/or rank as indicated on page 19 of the questionnaire. The completed questionnaires should be sent directly to Lisa Deener. In addition to providing the 1993 questionnaire, this memorandum serves as an advance notice of changes for the 1994 questionnaire. We plan to revise the following question in the PUBLIC FINANCIAL DISCLOSURE, CONFIDENTIAL FINANCIAL DISCLOSURE, and SPECIAL GOVERNMENT EMPLOYEES sections to read:  If the number of required financial disclosure reports is not the same as the number of reports actually filed, please provide the appropriate number(s) for the reason(s) given. In closing, the 1993 questionnaire is due on or before February 1, 1994. The data must cover January 1, 1993 through December 31, 1993. Failure to timely file the questionnaire may be cause for OGE to invoke its corrective action authority (5 C.F.R. § 2638.602(b)).
